     ADVERTISEMENT FOR BIDS

    Date:  November 24, 2009    Revised: December 7, 2009

     Notice is hereby given that the Owner, City of Collinsville, will accept bids for the construction of the Project identified Collinsville City Park and located on South 19th Street and Veterans Drive .

     All bids must be in accordance with the Contract Documents prepared and issued by LandPlan Consultants, Inc. located at  1110 West 23rd St. Tulsa , Oklahoma 74107 .

     The following is a general description of the Project:

    Construct the Collinsville City Park including spray pad, skate park,
    basketball court, playgrounds, grading, drainage, surfacing, lighting,
    landscaping and other related items.

     Bids will be accepted on a Unit Price basis, and when applicable, includes additive and/or deductive alternates.

     Bids will be received at LandPlan Consultants, 1110 West 23rd Street Tulsa , Oklahoma 74107 until 2:00 PM local time January 6, 2010.  Bids received after the above time and date will be rejected.

    At the above time and date, bids that have been duly received will be opened and read aloud. LandPlan Consultants, Inc. will review the bids and make a recommendation. A recommendation for the most successful bidder will then be presented to the Collinsville City Council. 

    A “Bid Bond” or other bid security in the amount of 5 percent of the bid amount must accompany the submitted bid.

     No bids may be withdrawn within a period of sixty (60) days after the date established for opening bids.

     The successful Bidder will be required to furnish a Performance Bond and a Payment Bond, each for 100 percent of the contract amount, as security for the faithful performance of the work and the payment of all bills and obligations arising from the performance of the work.

     Copies of Bidding Documents may be obtained in person after 8:00 a.m. local time on November 24, 2009 at LandPlan Consultants   1110 West 23rd Street , Tulsa , Oklahoma 74107 for a price of $40.00.  Bidding documents will be mailed upon receipt of a check in the amount of $60.00. No partial sets will be issued and no refund will be provided for sets that may be returned. The phone number for Emily Elkins  at LandPlan Consultants is 918/746-1340.

     All bids, which shall include the costs for payment and performance bonds when required, shall remain binding on the Contractor for a period of 60 days after the bid date and time and the Owner reserves the right to accept any bid, bid alternate or any combination thereof, to reject any or all bids, or to waive any informalities in bids received where such acceptance, rejections, or waiver is considered to be in the best interest of the Owner.  All alternates requested on the bid form will be considered in the award of the Contract.

    A mandatory pre bid meeting conference will be held at Collinsville City Hall , 106 N. 12th Street in Collinsville , Oklahoma at 2:00 PM local time, December 17, 2009.  It is mandatory that each Contractor have a representative in attendance to bid the project.

       

    ADDENDUM #1   12-07-09

      Collinsville City Park

    The following items shall become a part of the contract documents for the above referenced project:

    1.                  Document 00030 has been revised as follows:

      The original statement of:

    “Bids will be received at Collinsville City Hall Commission Chambers, 106 N. 12th Street Collinsville , Oklahoma 74021 until 2:00 PM local time December 17, 2009.  Bids received after the above time and date will be rejected. “

    Has been revised as follows:

    “Bids will be received at Collinsville City Hall Commission Chambers, 106 N. 12th Street Collinsville , Oklahoma 74021 until 2:00 PM local time January 6, 2010.  Bids received after the above time and date will be rejected. “

    The original statement of:

      ADDENDUM #2    12-22-09

    Collinsville City Park

     The following items shall become a part of the contract documents for the above referenced project:

     1.     Add the following to Document 00100 Instruction to Bidders, section 14:

             14.2  The contractor will be permitted to subcontract a portion of the work but shall perform with his or her own organization, work amounting to not less that fifty (50) percent of the contract amount.

           2.     All bid forms must be notarized near the signature line on sheet 00300-3.

     3.     Revise Document 02800 Playground Equipment Installation, Section 2.02 H to read as follows:

             Following completion of playground equipment installation, the contractor shall pay for and arrange for a playground Safety Inspector certified by National Recreation and Park Association to inspect the playground and file a report to the Owner, Contractor and Landscape Architect. The Contractor prior to final acceptance shall address safety concerns noted in the report.

               4.          Add to the end of the General Construction notes
                      
          on sheet 3 the following note:

    Heavy truck traffic during construction shall be limited to the east entrance at Nineteenth Street and approximately 465’ to the east near the proposed 5 car parking near the skate pad.  Any damage to existing road pavement outside this area due to heavy trucks/vehicles by the Contractor shall be repaired or replaced at the Contractor’s expense as directed by the Landscape Architect.  Heavy truck traffic shall not be allowed to enter and/or exit from the north entrance on West Spring Street or west entrance at Fifteenth Street to the park. 

     5.      Add note to Sheet 34:

    The Contractor shall provide all sleeving required for utilities that will run through the concrete footing that he will be providing. This will include sleeves sized to accommodate 4” SCH 40 PVC sanitary sewer, 1 ˝” SCH 40 PVC water line, and 2-1” steel electrical conduits.

    ADDENDUM #3   12-31-09

    Collinsville City Park  

    The following items shall become a part of the contract documents for the above referenced project:

     1.     Substitute the enclosed revised page into Document 00300 Bid Form, page 00300-7 that adds the pay item at the bottom of the page for Retaining Wall. This page must be included in the bid submittal.

     2.     Sheet 2 of drawings:

    Add pay item for Retaining Wall at 31 lineal feet. Add pay notes 59 and 73 to this item.

    3.         Sheet 3 of drawings:

                      Add pay note 73 which will read as follows:

                   Shall include the cost of all materials and labor to install all items shown on retaining wall detail A on sheet 24 including but not limited to stone, cap, cmu block, rebar, mortar, concrete footing, excavation and backfill. Drain pipe and aggregate behind wall covered under pay item 4” perforated schedule 40 drain line.

           4.        Add the following to Sheet 2 of the drawings at the end of pay note #13 that is attributed to the pay item for Installation of Modular Skate Park Equipment for both base bid and alternate:

    Cost shall include all assembly of skate park equipment pieces per the manufacturer’s written instructions. Contact local sales representative, Bryan Montgomery of ACS Playgrounds for more information on the assembly. Phone number is 405/406-9670.

    5.          See Project Manual, Section 02807 Skate Park Equipment Specifications, page 02807-1, Part 1 General, 1.01 Submittals, items A through H. Delete this item entirely. The skate park equipment is being provided by the City.
